How to apply to the course?

Sign up using this application ONLINE FORM.

The form will take a few minutes to complete and you will need your identification documents.

Condition

After filling out the online application form, there are two other admission conditions:

1. Send the required paperwork listed below to Study Division

2. Pay tuition by December 15th 2024.

Once these conditions have been met, you will receive a statement of acceptance for the Premedical course.

Required documents that must be sent

Nostrification

Provide your secondary school leaving certificate (authenticated in the form of an apostille or superlegalisation according to your country: forms of authentication) and transcripts of the upper secondary school years. These documents must be submitted with a certified translation into English or Czech.

What to do:

1) have the original of your high-school leaving certificate and trancript of grades from each study year

2) take a notary verified copy of it (made by a Czech or foreign notary or in the Czech Republic available at CheckPoint - post office)

3) get an official translation to Czech or English language

4) get it authenticated in form of Apostille or Superlegalisation (https://www.lf3.cuni.cz/3LFEN-812-version1-nostrification_manual_eng.pdf)

5) Send the documents in hard copy by post or deliver personally to the faculty address, written below.

Medical report

Medical report certify that you are physically and mentally fit to start the course. Print the report and have it filled out by your doctor.
