Current Advanced Life Support (ALS) guidelines for cardiopulmonary resuscitation (CPR) are not established for two-members rescue teams. However, the emergency medical service (EMS) teams in the Czech Republic most often have a two-member team of rapid paramedic assistance or rapid medical assistance in the rendezvous system (rendez vous). These EMS teams are not often deployed to sudden cardiac arrests at the same time. The arrival of the first EMS team on the scene may precede the addition of the team to four rescuers by several minutes. However, in this situation, there is a need to perform several ALS actions and not just basic aid consisting of continuous chest compressions, early defibrillation and ventilation of the patient. However, there is minimal data on the adaptation of ALS recommendations for CPR including the two-member team. Currently, each EMS chooses its own modified procedure. The aim of postgraduate studies should be for the student to be able to carry out an review of the current state of the issue (scoping review level), then, after identifying knowledge gaps, select the areas and objectives for the following randomized simulation study, which will evaluate the optimal ALS procedures for two-member teams of rescuers. The conducting of a randomized simulation studies will be preceded by a national survey among EMS, how they treat the situation and what are their opinions or adaptations of the ALS protocol for a two-member EMT teams. Randomized simulated studies will be divided into several arms according to the type of sudden cardiac arrest. In addition to the aforementioned, the outcome of the study and the subject of the dissertation should also be a recommendation for practice, especially for prehospital emergency care and EMS, but transferred also for other medical facilities with a smaller amount or in case of unavailability of staff. |
