Do not forget fill in all information and check off term of the examination in Prague!

The applicant will not be admitted without the presented requirements.

1. Application Form for Admission to study in English - General Medicine.

2. Verified copy of high school leaving certificate certified with an Apostille, by the Ministry of Foreign Affairs or Ministry of Education and the Czech Embassy of the country you studied in and official transcript including all marks/grades and number of hours.

3. Photocopy of the 1st page of passport (write on the back side that you agree to provide your personal data for the purposes of admission and sign it, please).

4. One passport size photo.

5. Applicants are requested to mail the above documents to the faculty address and pay the admission fee of 830,- CZK or equivalent amount in Euro (non-refundable)
